The card gives her own forename („Anny”, in the genitive), her married surname in the feminine form („Sztelegowskiej”) and her maiden surname in the formula „z Staniszewskich” — hence the status: „z domu Staniszewska”, née Staniszewska, on the model of Franciszka née Głuchowska Nurzyńska (Szaniawy Matysy) and Katarzyna née Karwowska Radzikowska.
